Option 1 Waiver (Pick-Up in Person)

I agree to pick up my TRIP cards on Friday from 8:00am -3:30pm in the Upper School Office. I understand that if I do not, they will be place in a secure location and unavailable until the following week.

Option 2 Waiver (Backpack Distribution)

I assume all responsibility in choosing the backpack distribution method. I will consider the age and maturity of my child, how he/she gets home from school and the potential situations where the backpack is unattended. I am aware that there are possible consequences; gift cards have a cash equivalency. The MCA TRIP program and MCA will not be responsible for lost or stolen gift cards. Orders will be sent home in backpacks on Fridays(on Thursday if no school on Friday)

Option 3 Waiver (MS/HS Pick-Up)

I understand that my student will be called to the Upper School Office to pick up at the end of the day on Friday. I understand that if the order is not picked up it will be placed in a secure location and unavailable until the following week. By choosing to send my gift card order home with my child, I will not hold MCA or the MCA TRIP Program responsible for lost, stolen, or misplaced TRIP orders once my child has been given my packet and delivery has been documented.
